Improving health and survival of mothers and newborns depends on the delivery of evidence-based cost effective interventions through a skilled care provider. Evidence suggests that midwives are compassionate skilled care providers to mothers, and ensuring these skilled human resources is a key to progress towards Millennium Development Goals 4 and 5. With this in mind, this article analyses the state of midwifery in Nepal and what strategies are needed to ensuretheir accessibility and availability. In Nepal, as a result of community mobilization through Female Community Health Volunteers for birth preparedness and complication readiness, there has been a demand for skilled care at birth. However the supply of skilled birth attendants has been inadequate both in terms of number as well as quality of care.&#xD;

Sustainable environmental management is of great concern due to rapidly growing number of health care institutions in Nepal. It is mandatory to regulate and develop a practice for impact assessment due to establishment and operation of health care facilities of Nepal through userâ€™s friendly procedure. This study was carried out to review Initial Environmental Examination (IEE) process and to recommend appropriate IEE study guideline for establishment and operation of health care facilities in Nepal. Primary data were collected through consultation and interaction with domain experts and concerned stakeholders. Secondary sources of information include review of available literature, browsing information from internet, review of IEE studies of developmental projects of different sectors. Review of IEE documents showed that potential effects of the health care projects on environment are inadequately mentioned and whenever mentioned not applied in operational stages. A clear and userâ€™s friendly IEE guideline for establishment and operation health care facilities has been proposed to conduct IEE study in Nepal.&#xD;

The management of health care waste has become an urgent need to safeguard the public health from adverse effects caused due to improper management of healthcare waste. Healthcare waste management continues to present an array of challenges for developing countries, including Nepal. As the demand for healthcare facilities increases, there is also an increase in waste generation from these facilities. This situation requires an organized system of healthcare waste management to curb both public and occupational health risks. The aim of this paper is to explore the current situation of health care waste management practices in Nepal and its possible impacts on human health as well as recommend the best practices.&#xD;

Healthcare governance is a framework for improving quality of care and access by increasing accountability and promoting transparency for the excellent outcome of healthcare, shared learning and sharing. Healthcare governance plays a vital role in improving patientsâ€™ experiences, decreasing disparities in healthcare and shared learning from the experiences by promoting openness and culture of accountability. Service usersâ€™ involvement, in the process of health planning and delivery, is an opportunity to improve their experience. It is now time to introduce strategies and policies in Nepal to implement the concept of healthcare governance to improve the quality of care and access.&#xD;
